**Mō te tūnga **| **About the Role**

Our team is a professional and highly skilled Social Work team, who display a positive attitude and enthusiasm for health social work. We enjoy the challenge of delivering patient centred care in a fluid and dynamic multidisciplinary environment. We respond to a diverse range of health and social needs, no two days are the same. We provide 24-hour social work service 7 days a week, as part of a multidisciplinary team who take a holistic approach to patient care. We work across all areas of the hospital with the multidisciplinary team to support patients - and their whānau's health journey from admission to discharge. These are rostered and rotating positions.

**Ko wai koe | About You**

You will be joining our supportive and enthusiastic Social Work team. You will thrive in an unpredictable and at fast paced environment.

In this role, you will be responsible for:

- Assessment, monitoring and support of clients within the acute social work service in accordance with ANZASW Code of Ethics and Standards of Practice.
- You will need strong assessment and triaging skills to meet patient needs.
- Attend and participate in social work team and departmental activities as well as staff meetings.
- Participate in service development and quality assurance initiative/processes.

**Ngā pūkenga me ngā wheako | Skills and Experience**

Desired skills will include:

- A passion for working with people and their whānau/families.
- Excellent communication skills and the ability to relate to people from all walks of life.
- Experience within, Elderly, Care and Protection, Family Harm, and Trauma, is desirable but not essential.
- Current Annual Practicing Certificate issued under the Social Worker - s Registration Act (2003) is essential.

This role will see you working alongside different multidisciplinary teams so being a team player who can quickly adapt will be essential to your success. Our service is committed to the professional development of our staff and offers regular ongoing development opportunities.
