**The Opportunity**

An exciting opportunity has become available for a Clinical Nurse Specialist (CNS) to join the Acute Stroke Service.

This is a permanent part time position working 20 hours per week - 0.5 FTE.

The CNS will support the existing CNS in the provision of expert nursing support to health professionals in the care and treatment of patients who have suffered an acute stroke or TIA. The CNS will role model effective evidenced based practice and support the coordination of care through collaborative workings of multidisciplinary teams across the secondary health care settings.

The CNS role is a dynamic position, which also guides nursing and system interventions of acute stroke care to improve patient outcomes, whilst providing support and information to patients and Whanau.

This is an excellent opportunity for an individual who has the necessary clinical and interpersonal skills to work autonomously within the challenges of the acute health care settings to best support patient-centered care.

**Key Responsibilities**:

- _Excellence in clinical practice_ - assessment, planning, implementation and evaluation of patient centered care and support of Whanau to improve care outcomes and resource management.
- _Leadership and consultation _- provide support and advanced nursing expertise to develop knowledge and skills of health care professionals in patient care through formal and informal education.
- _Research, quality improvement, innovation and change _- actively participates in support of stroke data collection, audit systems, reporting and policy development and implementation, governance and networking to develop local and national stroke services.
- _Professional development _- The CNS translates advanced nursing standards, expert clinical skills, critical thinking, and advanced knowledge to support others in the provision of evidence based acute stroke care.
