As our Outreach Facilitator, you will be responsible for the flow of information and the planning process for our clients who are in public hospital. Working collaboratively with hospital staff, you will ensure our clients are receiving the right service and support in order to achieve the desired outcomes for individuals and their whanau.
Taikura Trust is the region's single facilitator of needs assessment and service coordination of support services for people under 65 years, who have a physical, intellectual or sensory impairment, or autism. Above all, Taikura Trust is about bringing certainty to the lives of the people we support and walking the journey with children and families to live the lives they choose.
**About the role**
- Conduct professional needs assessments to identify and prioritise support needs for clients based in hospital.
- Drive the communication of relevant information to clients, their whanau, hospital and rehab staff to ensure all parties work together to achieve client goals.
- Maintain high quality documentation and planning
- Stay current in the Health & Disability Sector changes, maintaining specialist knowledge and understanding of best practice and new initiatives. Uphold Taikura values, ensuring respect and inclusivity are modelled in all aspects of the role
**About you**
- Solid experience working in the health / allied health / disability sector, and/or a related qualification. Clinical knowledge and/or experience in the hospital environment is an advantage.
- Proven experience in coordinating natural and formal supports and services for people with a range of physical, intellectual and sensory needs
- Strong interpersonal skills, and able to connect to a range of people
- Able to travel to public hospitals and health facilities, to conduct needs assessments Passionate about improving the lives of those living with disabilities and commitment to Taikura's values.
