Based at Princess Margaret Hospital, our North/East Community
Services Team is a large interdisciplinary team working with clients in the
community with a goal of maintaining patient independence. We offer assessment,
treatment and rehabilitation of older persons.
Ko wai koe | About You;

In this role, you will be responsible for:

  • The Case Management portion of the role will consist of the
oversight and management of a varying case portfolio;

  • Completing assessments of consumers and formulating interventions
to meet individual needs in collaboration with our multi-disciplinary team

(MDT)

Your skills will include:


  • Experience in Older Persons Health would be
advantageous;

  • The ability to triage and prioritize your patients appropriately in
a timely manner is essential;

  • A current NZ driver s license is essential;
  • New Zealand Registration as an Occupational Therapist with Annual
Practicing certificate is essential.
