**Mō te tūnga - About the Role**

We have an exciting opportunity to join the supportive Occupational Therapy team at Christchurch Hospital. We are looking for a suitably experienced NZ Registered Occupational Therapist to join us on a permanent basis as part of our Community Home Visit team. You will have a minimum of 2 years relevant clinical experience with sound clinical skills in occupational performance assessment and problem-solving skills. Experience working in the community would be an advantage but is not essential.

This role is a permanent role (40 hours per week), working Monday to Friday. It is also expected that you will contribute to the weekend roster for the in-patient wards.

**Ko wai koe | About You**
- In this role, you will be responsible for:_
- Working with patients in their own homes following a recent acute admission to Christchurch Hospital, to support their discharge back to community living.
- Be providing pre-discharge home visit assessments as clinically required.
- Supporting your colleagues across the team with complex discharge planning
- Working with patients from diverse social and cultural backgrounds and working collaboratively with whanau, the wider multidisciplinary team to ensure the best outcomes;
- Identifying and contributing towards service improvement initiatives.
- Being involved in transfer and discharge planning.
- Mentoring and supervision of other colleagues within the wider Occupational Therapy service
- As part of the team you will also contribute to the weekend roster at Christchurch Hospital
- There will also be the opportunity to upskill in acute in-patient working as needed/wanted

- Comprehensive clinical assessment and problem-solving skills,
- Ability to work autonomously and collaboratively, to develop positive interpersonal relationships, and work flexibly within the inter-disciplinary team, with a team player attitude
- Excellent communication and interpersonal skills, to liaise effectively with health professionals both within the in-patient services and in the community
- Strong clinical reasoning skills and use of evidence informed practice
- A sound knowledge of the role that adaptive equipment and environment modifications can play in supporting people - s occupational performance
- A knowledge of the Enable NZ and ACC funding processes
- Effective time management and organisational skills
- Strong prioritisation skills and experience with complex discharge planning
- Registration with the NZ Occupational Therapy Board and current Annual Practicing Certificate (or eligibility to obtain);
- A positive attitude and enthusiasm for this area of work;
- A full clean drivers licence is essential
