Access Community Health has a full-time vacancy for a Registered Health Professional (RHP.) We're looking for someone who shares our dedication - supporting the elderly, people with disability and people rehabilitating from injury and illness to live independently in their own homes and communities.


What does an RHP do?


We carry out social and clinical assessments, develop and oversee support plans and mentor and verify the competence of our support workers.

We work alongside other RHP's in the wider health community to ensure the best outcome for our clients.

Our RHP's work autonomously in the community, while being part of a dynamic team working in a supportive environment.

What are we looking for?

  • RHP with a current practising certificate
  • Experience in a communitybased role is highly desirable
  • Someone with finely honed assessment skills
  • Excellent verbal communication skills, coupled with a caring nature and the ability to connect and interact within a multicultural environment
  • A team player who can build and maintain professional relationships
  • The ability to selfmanage, effectively plan and organise, establish priorities and meet deadlines in a demanding role with a busy workload
  • Understanding of the responsive model of support and a client driven approach to support
  • Intermediate level computer and online information recording skills
  • A current full driver's license is essential
