We have an exciting opportunity for a Nurse Coordinator - Locality Hubs to join our compassionate team at the Mangere clinic.

This role involves coordinating outpatient clinics, which serves a vibrant and diverse population. Including the largest population of Pacifica people in the district.


As a senior nurse in this role, you will be the key outpatient contact for service provision for ambulatory care patients.

You will work closely with the services utilising the Hub to develop new and innovative models of care that maximise nursing input and meets customer expectations.

You will actively contribute to new service opportunities, promoting inclusivity, and ensuring seamless clinic operations.


In this role you will utilise general and specialty knowledge along with sound nursing judgement to assess the health needs of our patients and support the multidisciplinary team (MDT) to provide patient centred care.

You will support both medical and nursing clinics within the hubs to provide community-based care.


The role requires the ability to practice both independently and in collaboration with other health professionals to perform nursing functions.

Patients will have a variety of acute and/or complex needs - direction and delegation to other team members will be a key part of this role (Patient Care Assistant, EN and Health Care Assistant workforce)


To thrive in this role, you should demonstrate practice at an expert or proficient level on the PDRP scale, coupled with outstanding communication, and creative problem-solving skills.

Experience in clinic-based care and coordination would be a distinct advantage.

Essential:

  • A strong background of 35 years recent clinical experience in a clinicbased context.
  • Postgraduate certificate qualification or equivalent in the relevant clinical field.
  • Proven experience in creative problemsolving and conflict management.
  • Current Proficient or Senior Nurse PDRP

Desired:

  • Postgraduate diploma showcasing your commitment to professional growth.
  • Performing at an Expert or Senior Nurse Level on the PDRP scale, reflecting your dedication to excellence and equity.
