Te Puna Hauora is a Kaupapa Māori Whanau Ora, Health & Social Services provider based on the North Shore. We provide high quality, professional yet accessible & affordable health care and support to Whanau in the surrounding communities. Our integrated wrap around services includes GP Clinic, Tamariki Ora Team, Disease State Management Team, Community/Whanau Support and Mental Health Teams, Whanau Ora, Pharmacy, Dental Clinic and more.
We are currently seeking a reliable, experienced & enthusiastic Healthy Lifestyles Facilitator to join our Team working with Whanau in the community.
The role of Healthy Lifestyles Facilitator is to:
· To develop and implement the Te Puna Hauora Healthy Lifestyles IMAP model with Low Risk Tangata / whānau:
· Work alongside Practice Nurses and DSM nurses in supporting identified tangata / whānau to manage their diets and exercise plans.
· Organise group and individual sessions with flexibility on times.
· Prepare and monitor diet and exercise plans with tangata / whānau at a time and place convenient to them e.g. Home, workplace or Te Puna Hauora
The successful applicant must be prepared to gain a good understanding of the Kaupapa & policies of the Te Puna Hauora client led model & respect the rights of the Whanau at all times. Communication & organisational skills are essential, and you must have the ability to establish strong working relationships with a wide range of people.
The successful applicant will have:
· Relevant Qualification in Health, Personal Training etc
· Current Full NZ Driver Licence
· Ability to relate to people with sensitivity, caring and patience.
· Excellent written and oral communication skills
· Ability to speak Te Reo would be an advantage
Applicants must be eligible to work in New Zealand.
