Job Description

  • Maintain a holistic understanding of the independent resident group; and document and monitor the current allied health services already being provided to residents.
  • Identify residents who may require the support of external allied health or community services, make the appropriate referrals, and follow up to ensure resident health and wellbeing needs are being met.
  • Connect with residents' existing support networks, including their spouse and/or family members, as required and with prior permission.
  • Ensure Village Manager is aware of any residents who may require a transfer to a higher level of care.
  • Engage and seek opportunities in the resident wellness meetings to promote the Attitude of Living Well within the independent resident community.
  • Participate in and provide training, in-service education, and meetings as required for residents.
  • Provide agreed clinical services if appropriate, and within professional scope and credentials, and under appropriate supervision (e.g. immunisation clinics). The Village Liaison is not responsible for undertaking and executing clinical assessments or treatments.

Qualifications

  • Relevant tertiary qualification, professional qualification and/or a minimum of 3 years' community-based heath or social service sector experience
  • Previous aged care sector experience (may include needs assessment or similar) and passion for engaging and empowering older people to lead life their way
  • Strong interpersonal and influencing skills, including the ability to achieve goals with others
  • Understanding of and commitment to person-centered care and Arvida's Attitude of Living Well philosophy
  • Self-motivated with excellent organisational, time management, and problem-solving skills
  • Strong community liaison and promotion skills
  • NZ Full Driver' License
  • NZ First Aid Certificate
  • CPR Certificate
