About the Role

We have an exciting opportunity for an experienced Nurse Practitioner Diabetes to join our Long Term Conditions team on a permanent part-time basis (0.8 FTE).

The primary purpose of this role is to:

  • Work both autonomously and collaboratively with other health professionals to promote health, prevent disease, and improve access and outcomes for people living with complex long-term conditions with a specific focus on adult diabetic patients at high risk of early kidney disease.
  • Deliver a holistic, whānau-centred, and culturally responsive approach to care, particularly supporting Māori and Pacific communities.
  • Lead and manage episodes of care for this patient group, partnering with health consumers and their whānau to achieve the best outcomes.
  • Combine advanced nursing knowledge, diagnostic reasoning, and therapeutic expertise to provide patient-centred care including the diagnosis and management of both common and complex health conditions.
  • Act as a clinical leader, influencing service delivery and contributing to the continuous improvement of healthcare across settings and disciplines.

You will be part of the Long-Term Conditions team at Hawkes Bay Fallen Soldiers Memorial Hospital, working within an experienced multidisciplinary group of health professionals who are passionate about equitable healthcare and improving chronic disease outcomes.

About you

You will be an experienced and compassionate clinician who brings both clinical leadership and a passion for improving outcomes for people with diabetes and kidney disease.

To be successful in this role, you will have:

  • Current registration with the Nursing Council of New Zealand as a Nurse Practitioner, with a current Annual Practising Certificate.
  • Demonstrated experience in implementing Te Tiriti o Waitangi in action.
  • At least five years of recent clinical experience in Diabetes and Renal nursing.
  • Proven ability to work collaboratively within a multidisciplinary team.
  • Strong problem-solving, communication, and time management skills.
