Transition Support Worker and Trainer

**Healthvision is a lead provider of Specialist Homecare, Community Nursing, and Rehabilitation services throughout New Zealand.**
**We are looking for an energetic and adaptable Transition Support Worker and Trainer to join our dedicated Homecare team in Auckland. This is community-based work with variable working hours, as a Support Worker you may be required to work in the weekends.**
**This position is based in our Auckland head office in Greenlane and will be reporting to our Homecare Clinical Lead. You will need your own car and be prepared to travel out of Auckland, nationally as required, to assist with client transitions and provide training to Support Workers and facilitate new staff inductions.**
**Key Job Responsibilities**:
- **Assist the Registered Health Professionals (RHPs) to develop support plans**:
- **Support Client discharge from Medical Unit to home environment**:
- **Facilitate new Support Workers induction**:
- **Contribute to delivering annual training for Support Workers**:
- **Deliver client specific training around care delivery and use of equipment**:
- **Work in partnership with RHPs and Client Services Managers in providing training to Support Workers**
- **Experience in Homecare support for a minimum of 3 years**:
- **L4 Health & Wellbeing qualification (or equivalent), provided support to complex care clients**:
- **Able to train and assess others on the job, and in groups**:
- **Excellent communication Skills**:
- **Excellent time management skills**:
- **Experience in providing client-specific care and understanding of the concept of wrap-around services to influence outcomes**:
- **Proven documentation skills**:
- **Understanding of privacy, confidentiality, and safe storage of information**:
- **Full drivers licence and the right to work legally and permanently in New Zealand**:
- **Evidence of your vaccination status (as per the COVID-19 Public Health Response (Vaccinations) Order 2021 which specifies that all frontline workers must be vaccinated)**
