**About the Role**
The Hutt Valley MHAIDS Service has an exciting opportunity for an innovative Mātanga Tapuhi Nurse Practitioner (NP) position or a NP Candidacy to registration for this area of practice. The role is located within the Hutt South Community Mental Health Team in Lower Hutt.The Mātanga TapuhiNurse Practitioner (NP) - Mental Health is an autonomous practitioner who works independently and collaboratively with multidisciplinary teams (MDT) and providers to support the safe transition of adults and older people with complex mental health needs to the residential care providers. Tangata whaiora from the district and central region transitioning through Kelvin House from mental health and hospital speciality services require comprehensive assessment to enable best management for significant mental health and often complex physical health needs. The Mātanga Tapuhi NP role will support optimising care provision using an individual and population-based approach to collaboratively support healthcare needs for these vulnerable tangata whaiora with a diverse range of mental health conditions.
Referrals for this service may originate from rehabilitation, acute, or community pathways where recovery focused approaches are no longer appropriate and the individual would benefit from long term supportive care in a secure environment and working with the Lower Hutt South Community Mental Health Team.
As a Nurse Practitioner part of the role will be working closely with the team at this unit to provide clinical oversight of this cohort of clients. This can include being the MHA Responsible Clinician and providing MH care and support and will also be working within the Hutt CMHT to provide services for people with mental health need**s.**
**About the Team/Service Location**
The Local Adult Specialist Mental Health and Addiction Service is implementing a vision, principles and future model of care that is integrated, culturally responsive, equitable across the district and aligned with national and local directions. The team is a multidisciplinary team with service provision for people with mental health needs.
**About you**
- Will have a proven experience and evidence of advanced mental health nursing expertise.
- Be able to work as an autonomous practitioner who works independently and collaboratively with multidisciplinary teams (MDT)
- You will also be highly organised with exceptional interpersonal and relationship building skills,
- A thorough understanding of quality improvement and clinical processes,
- Thorough understanding of the Mental Health Act.
- Committed to achieving equitable outcomes for Maori whanau.
We are looking for a recognised advanced practice nurse that has a passion for ongoing learning and the ability to adapt to work effectively with different learning experiences. You will need:
- Experience in Mental Health nursing (minimum 3 years)
- Registered Nurse (RN) and a current practising certificate with no restrictions from the NCNZ
- Expert level on the PDRP or Senior Expert pathway with at least three
- four years relevant expertise and working at an advanced RN practice level
- A Clinical Masters or the postgraduate courses to meet the requirements to undertake the NP postgraduate practicum aligning to the national Nurse Practitioner Training Programme (NPTP).
