We have an exciting opportunity for an experienced nurse to join the engAGE Community Team as a Māori Gerontology CNS at Te Whatu Ora Te Matau a Māui. Become part of a dynamic and supportive team who are passionate about improving services for pakeke/ kaumātua in the local community. You will work closely with a dedicated team of Geriatricians, Gerontology CNS, Physiotherapists, Occupational Therapists, Social Workers, Dietitians and Speech and Language Therapists. You will have support and the opportunity to learn from and share knowledge with a diverse MDT.
You'll work in the community, using advanced clinical assessment skills to support older people to remain independent in their own homes for longer and also supporting those transitioning home from hospital. You'll be part of the award winning and internationally recognised engAGE model
- a multi-disciplinary team working in partnership with General Practice and other community service providers to come up with creative solutions for complex situations. You will work closely with partners in the Aged Residential Care sector to support and respond to older residents with complex medical situations.
We are looking for a Clinical Nurse Specialist with excellent team work skills, strong problem-solving skills and a creative “can do” attitude. This is a predominantly community-based role and you will be well supported by the engAGE Community Manager and Nursing Professional Leads.
This position provides variety and an opportunity to grow and develop expert nursing skills to further your nursing career. You will also have the opportunity to develop additional skills that cross traditional disciplinary boundaries by adopting an Interprofessional approach. The older persons service is expanding and working on new models of care. This is your opportunity to be part of these exciting changes
