**About the Role**
The Specialty Clinical Nurse (SCN) is a designated senior role. We have a 1 FTE permanent opportunity available based in Wellington. In this role, you will be responsible for improving health outcomes for Pacific patients admitted to hospital. Quality improvement is an important priority as is the patient experience. This role will provide support to clinical staff working with Pacific patients.
The SCN will respond to referrals from inpatient and outpatient services as well as promote access to Pacific community support services. The SCN will contribute to patient care through a shared care commitment to bring about improvements to enhance patient safety, self-management and improved outcomes. The SCN has responsibility to ensure their practice and the patient information/education provided is evidence based and specialty specific. The SCN role models cultural competence and supports a culturally informed clinical environment
**About the Team/Service/ Location**
The Pacific Health unit provides support to Pacific people admitted to hospital and also acts as a cultural resource for the hospital's inpatient services. While this role is primarily based in Wellington, it also covers Kenepuru and support Hutt Pacific team when required. This multi-disciplinary service provides support and advocacy to Pacific patients and their families from admission to discharge. Pacific priorities for the Unit include, but are not limited to the following:
- Address health literacy
- Improve the health of Pacific children and young people
- Reduce Ambulatory Sensitive Hospitalisation (ASH) rates
- Reduce ‘did not attend' (DNA) rates
- Prevent and reduce chronic disease and improve self-management of long-term conditions
- Reduce amenable mortality
- Reduce obesity
- Improve oral health
- Improve access and utilisation of inpatient services
- Improve discharge planning and facilitate access to Pacific providers in the community
**About you**
As a Specialty clinical nurse, you'll be able to work collaboratively across the sector and organisational boundaries to influence the nursing contribution to better outcomes for our whānau and communities.
- Registration with the Nursing Council of New Zealand as an RN
- Current practising certificate
- Relevant post-graduate qualification or working towards this
- Proven ability with information technology
- High quality care for the patient/client/family
- Ensuring that they follow through on their work
- Delivering identified outcomes
- Expert speciality knowledge
- Management and leadership
