**(Scope of Practice: Allied Health Professional - Physiotherapist, Occupational Therapist, Social Worker
- Advanced Practitioner)**

An exciting opportunity has arisen for practitioners with minimum 3 years post registration and relevant clinical experience to provide rehabilitation as part of the Post-Acute Community Rehabilitation Service. The social work role requires 5 years' experience.

The Post-Acute Community Rehabilitation Service (PACR) is a collective of services primarily dedicated to supporting clients immediately post their acute care episode. The PACR service aims to deliver the following initiatives consistently across the four localities (Palmerston North, Feilding, Tararua and Horowhenua) in the MidCentral region:

- Rehabilitation in the Home (ACC Non-Acute rehabilitation)
- Post-acute Community Stroke Rehabilitation
- Health Recovery bed therapy interventions to support slow stream rehab prior to returning home or receiving inpatient rehabilitation services.
- Transitional care beds to support Protection of Personal and Property Rights (PPP&R) process. The social worker will have a specific role in relation to PPPR.
- Voice therapy to support stroke rehabilitation, Parkinson disease and gender affirming intervention

This position would suit Registered Allied Health Professionals. Applicants will need to have a current annual practicing certificate. A current driver's licence is essential.
