**This is a full-time role**
**Main responsibilities**:
- Deliver tailored support plans under the supervision of the therapist.
- Assist clients and whanau to establish skills and routines.
- Link clients and whanau to relevant community resources and supports.
- Identify barriers and actively support the client by problem-solving to achieve a solution.

**About you**:
- Hold or be working towards a New Zealand Certificate in Health and Wellbeing from an NZQA accredited provider at Level 4 or above. Desirable but not essential.
- Annual Practicing Certificate for relevant qualification if applicable.
- Full NZ driver's license for community rehabilitation
- Ability to adapt communication style to build rapport and establish credibility with a broad range of customers across different industry types.
- Excellent interpersonal skills and ability to work within a team.
