**About the Role**

Supportlinks is the Needs Assessment and Service Coordination (NASC) service which provides access to support services in the Te Pae Hauora o Ruahine o Tararua MidCentral district for people with acute/short term needs, people who have long term health conditions, and older people/kaumātua with age related health conditions.

Supportlinks work primarily with a community focus however also has a role within inpatient services supporting the clinical teams with discharge planning, and if needed facilitates access to support for a person when they are discharged.

- a full, current, driver's licence
