We are Te Whatu Ora, Te Toka Tumai, Auckland. We provide health and disability services to more than half a million people living in central Auckland, regional services for Northland and greater Auckland, and specialist national services for the whole of New Zealand. Our main sites are Auckland City Hospital, Greenlane Clinical Centre and Starship Children's Hospital, located in central Auckland.

Te whiwhinga mahi | The Opportunity
We are looking for a Pacific Clinical Nurse Specialist of Polynesian/Micronesian/Melanesian descent to join our expanding Hospital in the Home team providing care to patients in their home. With a focus on achieving health equity as a priority of the health and disability health sector. Eliminating barriers and improving the delivery of care services for Pacific patients and their families. The Pacific Clinical Nurse Specialist will work with Pacific people and communities to improve access to care services.
The Pacific Clinical Nurse Specialists will be supported by the wider network of the Pacific Health Team, who are family centred and committed in improving health outcomes and health equity for Pacific people.
Intermediate Care comprises of a virtual hospital ward which delivers hospital-level acute health care services, offering a safe alternative option to hospital care, utilising our Interim Care Programme within Aged Residential Care (20 beds) and HiTH (40 beds) within the patients home.
As a virtual ward within Te Toka Tumai, clinical safety is assured with expansion of the multidisciplinary team to include:
- Medical oversight
- Nursing, including Nurse Practitioner, Clinical Nurse Specialists and RN's
- Allied Health, including Physiotherapist, Occupational Therapist and Practice Supervisor
- Nurse Specialist Maori Health and Oranga Coordinators who are embedding Te Tiriti o Waitangi actions, delivering equity focused care and providing holistic care beyond the current diagnosis
- Pacific Health Clinical Nurse Specialists who are delivering equity focused care delivery
- Community Liaison Clinicians who in-reach across inpatient settings to facilitate discharging to any of the appropriate community pathways
- Social Worker who addresses the social determinants of health of patients and whānau
Within HiTH the team can visit patients up to twice a day and have the required skill set to administer infusions and medications, perform blood draws, undertake point-of-care testing, conduct virtual consultations, address unmet social needs, provide in-home rehabilitation and educate patients and whānau to improve self-directed care.”
The Intermediate Care team supports whānau choice and timely hospital discharge. It delivers 'Person and whānau-centred care which empowers everyone to manage their own health and wellbeing, giving people, their carer's and whānau meaningful control

Nga Pūkenga Motuhake | Important Skills
We are looking for Pacific Health Clinical Nurse Specialist or Registered Nurses with;
- A current NZ practicing certificate that have a passion to better health outcomes for Pacific people and communities.
- Minimum of 3-5 years experience working in the either the Community or hospital setting
- Working towards post graduate qualifications
- Currently a PDRP Proficient/Level 3 nurse or working towards Proficient
- An ability to build relationships with both clinical and non-clinical staff and contribute regularly to leadership meetings.
- A “can do” attitude that can work with and challenge the system to affect equitable change
- Demonstrated ability to engage / connect with our Pacifica Community
- Fluent in a Pacific language(s)
