**Kōrero mō te tūranga - About the role**

This role is worked primarily on the Health of the Older Persons wards at Kenepuru Hospital, Porirua. The social worker in the role works closely with the multi-disciplinary team to provide assessment, intervention and discharge planning for patients who have transferred from Wellington Hospital to Kenepuru Hospital for a period of recovery and MDT intervention. The social worker will also work with patients on the Surgical and Specialist Rehabilitation wards when required.

The Social Worker in this role will ideally have experience assisting clients, family and whanau to manage the impacts of trauma, injury, poor health and disability and to participate, to the greatest extent possible, in their normal roles and wider social systems.

Along with your social work expertise, you’ll have good communication skills and the ability to quickly build effective working relationships. You’ll also have good time management and prioritisation skills and be quick to pick up the intricacies of working in different clinical teams

The Allied Health Inpatient team comprises of Social Workers, Physiotherapists, Occupational Therapists, Speech Language Therapy, Dietitians and Allied Health Assistants. We work closely together, supporting our colleagues to achieve the best possible outcome for patients and their families while have fun and laughter along the way.

**Moū - About you**

The health social worker’s skill and experience will enable them to:

- Complete comprehensive psychosocial assessments
- Risk assessment and safety planning
- Discharge planning and support
- Assist individuals, families and whanau to manage the impact of injury, diagnosis and treatment
- Supportive counselling
- Advocacy
- Facilitating family meetings
- Knowledge and ability to work with situations of family violence especially in regard to older and vulnerable adults
- Demonstrate a commitment and understanding of the Treaty of Waitangi and to work positively in improving health outcomes for Maori
- Practical support
- Identify and respond to bereavement, grief and loss
- Liaise with and refer to relevant internal and external agencies and services
- Navigate and work within the legal requirements of health care and social work
