**Ngā Ratonga - Our services**

We are a small and dynamic whanau, based in Forrest hill. We are a residential service, that supports up to five tāngata whaiora, who have dual disabilities and an axis one mental health diagnosis.

Our service aims to support tāngata whaiora reach their goals and realise their potential within a home environment, but most importantly provide a safe and nurturing space where whānau feel supported to live their best life.

**He kōrero mō te tūranga - The role**

At Forrest Hill residential service we Aid the Transition Process - walk alongside whānau as they learn to live as independently as possible within the community.

Your daily/ ongoing activities with tāngata whaiora includes setting goals, supporting them in building and maintaining healthy relationships with their whanau, community, and iwi, but also encourage participation in learning daily life skills. Part of this role includes continence, personal cares and implementing best hygiene practices.

Your internal relationships will see you working collaboratively with clinician's, to ensure we are providing a safe nurturing space for tāngata whaiora, ensure key documentation and handovers are accurately recorded, but also become our health and safety representative.

This role is for two permanent staff members and one part-time permanent staff member. All roles work on rostered shifts Monday - Sunday 24/7 and work on a rotating roster.

**He kōrero mōu - About you**:

- Ideally have a Level 4 Mental Health Certificate or Level 4 Health and Wellbeing Qualification (or equivalent)
- Experience in the dual disability sector
- Experience with personal cares
- Must have a full New Zealand driver's licence
- Good time management and organisational skills
- Be culturally responsive and non-judgemental
- See potential in people and thrive in a challenging and ever-changing environment
- Have strong professional boundaries
- Excellent computer skills and be competent with administration/filing.
