**He Kōrero mō te tūranga - About the Role**

An exciting opportunity has arisen for a Social Worker who is passionate about health social work. This role will provide a professional and effective Social Work service for consumers of Te Whatu Ora, Hauora a Toi Bay of Plenty Renal Service. As this is a new role there will be elements of service development.

With no day being the same, you will have an opportunity to work with people of diverse cultures and ages to undertake biopsychosocial, risk and safety assessments and plan interventions. Utilising your crisis intervention skills, you will provide therapeutic support for the patient and whānau and facilitate connections with Te Whatu Ora services and external agencies.

To be successful in this role, you must be able to engage well with clients and their whānau and work in a client-centred practice. You will join our supportive, friendly and experienced team.

This is a permanent part-time position working 40 hours per fortnight.

**He Korero mōu - About You**
- Hold a NZ recognised Social Work qualification with SWRB registration
- Have NZ post graduate experience of at least 3 years. Preferably with previous experience working as a Health Social Worker
- Have experience working with family harm, alcohol and drug services and mental health
- Ideally have established knowledge and networks of local community based support services
- Have excellent communication and conflict management skills
- Produce timely, accurate written documentation
