**Locality Care Co-ordinator**

**40 hours per fortnight**
**Are you a skilled, healthcare professional with excellent organisation, communication, and relationship management skills?**
**Strong cultural awareness and the ability to work collaboratively in a team will also be key to this position**.
The purpose of the Locality Care Co-ordination Service is to provide support and leadership to teams in developing common objectives set by the person/whanau.
A primary function is to contribute towards establishing and embedding a care coordination approach by connecting Te Whatu Ora, NGO’s, community organisations, social services, and General Practices teams within the locality.

- receive a satisfactory police check
- be satisfactorily COVID-19 vaccinated in line with NBPH Policy
