He Kōrero mō te tūranga - About the Role
We have an exciting opportunity for a Social Worker/Case Manager with Mental Health experience to join our Mental Health Services for Older People (MHSOP) Community Team. The role will be part of our mahi of supporting Tangata Whaiora and their whānau in their journey to wellness.
You will work with the Tauranga community MHSOP services, as well as the Tauranga based inpatient MHSOP ward, health partners, referred clients, their families/whānau and the community to provide psychoeducational groups, one to one assessment and support, and provision of assessments to assist clients to maintain independence and safety.
The focus is to deliver high quality care to the growing population in the Bay of Plenty. At the heart of our services are our CARE values - Compassion, All-one-team, Responsive and Excellence. We have a commitment to working in partnership and collaboration with consumers, family/whanau and the community.
This is a 0.8FTE (32 hours per week) on a 12-month fixed-term position to cover parental leave.
He korero mōu - About You
The successful applicant will be required to have an excellent understanding of mental health conditions, the Mental Health Act, PPPR Act, understanding of capacity assessment work up, a range of therapeutic modalities, dementia/delirium care.
The successful applicant will have a minimum of 3 years post graduate mental health experience, preferably with experience in older person's mental health

The successful applicant will be an experienced mental health Social Worker who has a passion for working collaboratively with a wide range of people and services. Ideally you will have a minimum of 2 years of post-graduate mental health experience, preferably experience in older person's mental health.
To be successful in this role you will have an excellent understanding of mental health conditions, the Mental Health Act, PPPR Act, understanding of capacity assessment work up, a range of therapeutic modalities, dementia/delirium care.
