Care Coordinator

Our nationwide teams of over 4,000 support workers and registered nurses support people to remain active, safe, and independent in their own homes and communities. It represents over 350 community pharmacies under the Life Pharmacy and Unichem brands, 45 medical centers under The Doctors brand, specialized nursing services through Total Care Health and home healthcare through Access Community Health. As we take on new challenges and continue to support our staff across the regions, we are seeking a driven and self-motivated Care Coordinator to join our LNI/Wellington team in the Petone office. The Regional Care Coordinator role contributes to the delivery of our home-based support service by: - Coordinating, within a call management environment, home-based care and support services that enable Access clients to live as safely and as independently as possible in their homes and communities. - We are looking for people who share our dedication to work in support of the elderly, people with disability and people recuperating from injury and illness. Requirements are: - Fully vaccinated against COVID 19 - Commitment to providing excellent internal/external customer service. - Able to build and maintain successful relationships with all types of people including the ability to adjust communication style as necessary - Able to question broadly to understand individual and situational dynamics and to 'Read between the lines" - Naturally empathetic and caring but able to maintain professional focus and observe boundaries - Excellent interpersonal skills that enable the building of effective relationships across diverse groups, including negotiation and conflict resolution skills - Excellent general PC skills including the full Microsoft Package - Excellent organizational skills, particularly time management and prioritising - An eye for detail and an ability to maintain a high level of accuracy - Ability to think creatively and strategically and utilise problem-solving skills - Works well under pressure and multi-tasks across functions and activities Access Community Health is a leading provider of home-based healthcare and support.
