Pact is currently looking for a part time Health Coach/Community Support Worker - Access & Choice to support people to make positive change in their life.

The Role

The primary purpose of the Health Coach/Community Support Worker role is to work actively in partnership with a Health Improvement Practitioner (Registered Clinician), and along with the rest of the General Practice team to assist patients with health challenges to improve their general wellbeing.

This role has a service development component and is an exciting opportunity to be part of a positive new initiative in primary health. The idea of the role is to work two days as a health coach in practice and one day as a Support Worker in the community.

We are looking for someone with a passion to work alongside clients recovering from mental illness. You must be understanding, observant, non-judgemental and easily adaptable to a dynamic environment. We are seeking someone who will inspire, be a role model have excellent interpersonal skills and demonstrate resilience, integrity and professionalism. Your role will include promoting the well-being of clients, maintaining a therapeutic environment and supporting clients to achieve independence.

Based at a Medical Centre in Dunedin 3 days per week (48 hours per fortnight)
Shifts available: Monday - Wednesday from 8:30am - 5:00pm

**Desired Skills and Experience**

You must be an energetic person with a can do attitude. Previous professional experience in the mental health field will be an advantage. A level 4 or other relevant qualification in mental health is desirable but not essential. Otherwise, there is an expectation that you will be willing to start working towards obtaining a level 4 mental health qualification after your first year of employment.

*Recognised Health Coach training or a commitment to undertake this training. *Excellent communication and interpersonal skills. *The ability to relate and engage with a drivers range of people from different cultural backgrounds. *A full drivers license and computer skills are essential. *If on a work visa, this must be valid for at least twelve months
