**He Kōrero mō te tūranga - About the Role**
A position has become available in the Community Allied Health Services (CAHS) Team in Tauranga for a permanent full-time Social Worker with at least three years' experience working in a health-related setting.
The CAHS team is comprised of allied health assistants, dietitians, occupational therapists, physiotherapists, social workers, and speech language therapists. This role involves the provision of social work services to clients within their own home and community.
The position is based at the CAHS office in central Tauranga and covers a wide geographical area in the Western Bay of Plenty. The Allied Health team works alongside individuals with physical health needs using a client centred, enablement approach. The team's focus is to maximise client safety and functional independence, to enable them to achieve their wellness goals so they can remain living in their own home.
A key part of this role is establishing and utilising professional networks, then using these networks to problem solve and meet client's needs. Another is building a knowledge of health systems and services, then using this knowledge to assist clients to navigate these systems and services.

A successful applicant will have been able to demonstrate their ability to be a clear communicator, a builder of connections within the wider community and have excellent time management skills. You will be independently managing a caseload of varying complexity and completing assessments with an interprofessional lens. You will be working closely with other professions as well as directing an Allied Health Assistant.
