**Te whiwhinga mahi: The opportunity**
We are seeking a passionate and holistic superstar to join our extended care team. If you are a person passionate about supporting individuals and whānau on their hauora journey, enhancing their own capacity to take care of their wellbeing, then this could just be the role for you.
As a key member of the extended care team, with pharmacists and social workers, you will be contributing to the collective team knowledge and support for the wellbeing of our community.
This position is part time working 16 hours per week.
You will be based from either our New Plymouth or Hawera office (flexible depending on where you live) and be joining a great work environment - collaborative, flexible and future-focused. Pinnacle Midlands Health Network is committed to making sure our people reach their professional goals.

- relevant dietetic experience, particularly in a community setting
- a passion for improving patient health through evidence based best practice
- demonstrated knowledge and experience of Māori models of health and wellbeing
- a keen ability to develop and maintain key relationships within a multi-disciplinary team, external agencies and supports.
- registration with the NZ Dietitians Board
