**Gateway Assessment Coordinator - Part Time**

**About the Role**
- Health NZ Taranaki are seeking either a registered nurse, registered social worker or other allied health professional to work collaboratively across primary and secondary health services, community agencies and education facilities to coordinate Gateway assessments for children and young people referred from Oranga Tamariki. _
- The position is for 0.4 FTE which is 32 hours a fortnight_
- The Gateway Assessment Coordinator receives referrals, collates health and education information and schedules health assessments with a paediatrician. The coordinator works closely with Oranga Tamariki Social Workers and Families/Caregivers to facilitate the health assessment, by offering education regarding the process and a supportive environment. The coordinator works, as part of an MDT to facilitate an action plan following the assessment and oversees the submission of completed reports._
- The role requires strong administration systems and processes and the ability to manage conflicting priorities. A good knowledge of health, community and education services is necessary or the ability to develop that knowledge and the professional relationships required, to be successful._
- The Gateway Assessment Coordinator is required to track referrals against contracted volumes and provide quarterly reports to Oranga Tamariki. There is a Gateway Governance meeting that the coordinator attends, these are held quarterly._

**About the Team/Service/ Location**
- The Gateway Assessment Coordinator works as part of the Community Child Health Team and is based at the Child and Adolescent Community Centre at Taranaki Base Hospital. Gateway Assessments may be held at alternative locations, across the region, including community locations or a client’s home._
- The coordinator works closely with other Health NZ services and staff to prioritise assessments, advocate for services and act as a resource for other Health NZ staff, regarding child protection and support for some of the most vulnerable children and young people._

- The work requires strong administration skills, scheduling abilities and an attention to detail. _
