The ACC Non Acute Rehabilitation Service (NARS) Key Worker/Clinical Coordinator position is responsible for comprehensive case management for ACC NARS contract patients.

Key duties include:

- Scan all inpatients on a daily basis and identify potential NARS patients
- Case manage all suitable patients on the NARS contract
- Attend relevant multidisciplinary and referral meetings
- Liaise with external agencies/families and patient to make sure support is in place for patients on discharge

**He korero mōu - About You**

We're looking for a dedicated team player to join our team. The tasks you'll be expected to undertake will vary, so flexibility is key to this role. You need to be able to cope well with a high workload and have the capability and capacity to work unsupervised. There is a high level of documentation required to undertake the necessary ACC requirements to case manage patients on the non acute rehabilitation contract.

Key skills and qualifications:

- A current practice certificate in order to operate as Keyworker (Nursing/Allied Health) is required
- Post graduate Rehabilitation qualification and FIM/InterRai accreditation preferred
- Experience working within a regulatory environment
- Experience working in a health environment
