**About the Role**
Supportlinks is the Needs Assessment and Service Coordination (NASC) service which
provides access to support services in the Te Pae Hauora o Ruahine o Tararua MidCentral district for people with acute/short term needs, people who have long term health conditions, and older people/kaumātua with age related health conditions.
In this role, you will be part of the OPAL Community Services team, working across the MidCentral District, alongside a supportive transdisciplinary team consisting of geriatricians, occupational therapists, physiotherapists, social worker, Māori connectors, allied health assistants.

**About you**
- an ability to work within a changing health environment

- Experience and ability to work effectively within a trans-disciplinary team, utilising individual and professional skills.
- knowledge and working relationships with iwi/Māori providers in the district
- knowledge of community resources and networks
- skills to use a laptop and learn new assessment software

- an ability to communicate with people from a wide variety of backgrounds and cultures
- effective written, oral and interpersonal skills
- strong organisational and time management skills
- a full, current, driver's licence
