Community Health Coordination Director

We are seeking a motivated and experienced individual to fill the role of Community Health Coordination Director.
As a Community Health Coordination Director, you will be responsible for overseeing the coordination of community health services, including Supportlinks. You will work closely with stakeholders to ensure that services are delivered effectively and efficiently, and that patients receive the highest quality care.
The successful candidate will have a strong background in healthcare, including a professional health qualification, current Annual Practising Certificate, and clinical experience in disability settings. They will also possess leadership skills, functional knowledge of interRAI assessment, and a deep understanding of the New Zealand Health System.
