The Job

This exciting opportunity is for a Social Worker who is passionate about providing exceptional care and making a meaningful difference in the lives of others. As a key member of our Family Support Team, you will play a critical role in championing end-of-life care and advocating for our patients and their whānau.

Your Key Contributions

  • Provide expert social work services to patients and families, including assessment, intervention, and support.
  • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans.
  • Engage with community partners to promote palliative care awareness and education.

What You Need to Succeed

  • NZ Registered Social Worker with a current Annual Practicing Certificate.
  • Proven experience in social work, preferably in a palliative care setting.
  • Strong communication and interpersonal skills.
