The Job
This exciting opportunity is for a Social Worker who is passionate about providing exceptional care and making a meaningful difference in the lives of others. As a key member of our Family Support Team, you will play a critical role in championing end-of-life care and advocating for our patients and their whānau.
Your Key Contributions
- Provide expert social work services to patients and families, including assessment, intervention, and support.
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans.
- Engage with community partners to promote palliative care awareness and education.
What You Need to Succeed
- NZ Registered Social Worker with a current Annual Practicing Certificate.
- Proven experience in social work, preferably in a palliative care setting.
- Strong communication and interpersonal skills.
