About the Role
We are looking for an experienced palliative care nurse to join our clinical team at Hospice Ebop. As a Community Palliative Care Provider, you will be responsible for delivering high-quality, patient-centered care to individuals and families affected by life-limiting illnesses in the Eastern Bay of Plenty region.
Key Responsibilities:
- Conduct thorough assessments of patients and families to identify their unique needs and develop personalized care plans
- Collaborate with other healthcare professionals to ensure comprehensive and coordinated care
- Provide emotional support and education to patients and families, promoting independence and dignity throughout the end-of-life journey
Requirements:
- Minimum 5 years of experience as a registered nurse, with a strong background in palliative care
- Excellent communication and interpersonal skills
- Ability to work autonomously in a community setting, while also contributing to a dynamic and supportive team
