Your Key Responsibilities Will Include:

  • Conducting comprehensive health assessments for patients in their homes, including assessing their physical and emotional needs.
  • Developing individualized care plans based on patient needs, preferences, and clinical guidelines.
  • Regularly following up on patient progress and updating care plans accordingly.
  • Providing education and support to patients and their families regarding managing chronic conditions, medication management, and other health concerns.
  • Collaborating with healthcare providers for referrals to optimize patient care.
  • Accurately documenting patient conditions, treatments, and outcomes in the Service Plan (ISP).
  • Responding to patient and family inquiries regarding health concerns and providing guidance on home care best practices.
  • Advocating for patients to access community resources and services as needed.
  • Complying with relevant regulations and organizational policies related to nursing care.

Requirements:

  • RN with a current NZ Annual Practicing Certificate.
  • Minimum of 5 years of nursing experience, preferably in home care, geriatrics, or community health.
  • Ability to work independently and manage patient care in a home setting.
  • Strong communication and interpersonal skills.
  • IT literate.
  • Excellent organizational skills and attention to detail.
  • Able to maintain a compassionate and empathetic approach in challenging situations.
  • Current and clean driver's license and reliable transportation to travel to patient homes.
