About the Opportunity

As a Health Improvement Practitioner, you will be responsible for delivering evidence-based behavioural interventions that promote holistic well-being and support the prevention of mental and physical health conditions.

The position is central to supporting an integrated 'Model of Care,' promoting seamless collaboration between general practice, community NGOs, and specialist services.

Your care, empathy, and culturally responsive approach will guide patients toward improved health outcomes.

Key Responsibilities
  • Deliver evidence-based behavioural interventions in individual, group, and whānau settings
  • Collaborate with the GP team to support consultation-based services
  • Guide clients through available health services to improve overall well-being
  • Promote prevention, acute care, and chronic disease management through tailored interventions
  • Engage in continuous professional development
Requirements
  • Full Registration with the Nursing Council of New Zealand with current APC
  • Clinical experience in delivering mental health and psychological interventions
  • Familiarity with Te Reo me ōna tikanga and experience working in diverse Māori settings
  • Strong communication skills, with the ability to work with Māori, Pasifika, and youth communities
  • Excellent relationship management and organisational skills
