The Role

We are seeking a passionate Registered Nurse to join our dynamic team in the Gisborne community. As a Clinical Care Coordinator, you will leverage your nursing expertise, assessment skills, and professional judgment to provide top-notch community-based clinical support, education, and advocacy. Our clients have diverse needs, from injury recovery to long-term care, ensuring that no two days are ever the same. This is an opportunity to shine and grow in a supportive environment.

Your Responsibilities

  • Identify client needs using clinical assessment tools and develop individualised care/support plans to achieve positive outcomes.
  • Provide clients and their families/whanau with relevant information about their treatment and support, enabling informed decision-making.
  • Liaise with primary care and other services to ensure an integrated approach to delivering client services.
  • Demonstrate culturally responsive practices that respect a diverse range of beliefs, values, and practices.

Your Qualifications and Skills

  • Registered Nurse qualification with a current NZ practising certificate.
  • Strong clinical assessment skills and a desire to advance your career.
  • Outstanding interpersonal and time management skills.
  • Quick-thinking and adaptable in a dynamic, rewarding setting.
  • A warm and engaging personality with the ability to build instant rapport with clients and colleagues.
