**The Role:**

This is a shift work position covering after-hours shifts, assessing new referrals to the service and supporting community teams with any after-hours support required.

**Key Responsibilities:**

  • Assess patients in distress after hours
  • Develop and implement plans to support patients to achieve their goals
  • Work closely with community teams to provide support and engage with organizations

**Essential Skills and Qualifications:**

  • Degree-qualified Registered Nurse
  • Proficient or expert PDRP qualification
  • Good understanding of the Mental Health Act
