**Role Overview**

Matariki Hospital and Community is seeking a dedicated and skilled occupational therapist to join their team. As a Care Coordinator, you will provide high-quality assessment and intervention services to patients in the hospital and community settings.

The ideal candidate will have at least 12 months of post-qualification experience and be registered with the OT Board of New Zealand. You will work collaboratively with multidisciplinary teams, including physiotherapists, social workers, and allied health assistants, to provide holistic care to patients.

**Key Responsibilities**

  • Provide quality assessment and intervention services to patients in hospital and community settings
  • Manage caseloads, including stroke rehabilitation, and work with multidisciplinary teams to achieve patient goals

**Requirements**
