Care Coordinator Position

At Access Community Health, we are looking for a skilled and compassionate individual to join our team as a Care Coordinator. As a key member of our supportive and friendly team, you will play a vital role in delivering high-quality home-based care services to our clients.
The successful candidate will be responsible for rostering support workers across the region, collaborating with their team to create solutions and complete rosters. They will also develop and maintain strong relationships within the Access team, our clients, and their whānau.
To excel in this role, you will require excellent communication and problem-solving skills, as well as the ability to work effectively under pressure. If you are passionate about making a difference in your community and possess a commitment to Te Tiriti o Waitangi, we encourage you to apply.
- Roster support workers across the region to ensure seamless delivery of care services.
- Collaborate with the team to create solutions and complete rosters, ensuring effective time management and prioritization.
- Develop and maintain strong relationships within the Access team, our clients, and their whānau, fostering a culture of respect and empathy.
- Ensure adherence to policies and procedures, guaranteeing the best outcomes for our clients and support workers.
- Contribute to the smooth operation of the team, providing general administrative support as needed.
