Your Role
As a Person-Directed Support Specialist, you will be responsible for delivering high-quality support services to individuals with disabilities or long-term health needs. You will work collaboratively with clients, families, and other stakeholders to develop and implement personalized support plans that promote independence, inclusion, and empowerment.

Key Responsibilities
- Conduct comprehensive assessments to understand client needs and goals
- Develop and implement personalized support plans that promote client independence and inclusion
- Establish and maintain strong relationships with clients, families, and other stakeholders
- Collaborate with internal teams to ensure seamless delivery of support services
- Monitor and evaluate client progress, making adjustments to support plans as needed
Requirements
- Level 5 qualification or above (essential)
- Empathy and passion for supporting individuals with disabilities or health needs
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and conflict resolution skills
