About the Role:

This senior nursing position provides an exciting opportunity to lead a team of healthcare professionals in delivering home and community support services. As a key member of our team, you will be responsible for assessing patients' needs, coordinating care plans, and collaborating with other healthcare stakeholders to achieve optimal outcomes.

Key Responsibilities:

  • Assess patients' needs and develop individualised care plans
  • Coordinate care delivery with allied health professionals and community service providers
  • Maintain accurate records and reports, adhering to regulatory requirements
  • Collaborate with multidisciplinary teams to ensure seamless care transitions
  • Contribute to quality improvement initiatives, promoting best practices in patient care

Required Skills and Qualifications:

  • Minimum 2 years of experience in nursing, preferably in home and community care
  • Strong leadership and coordination skills, with ability to manage multiple priorities
  • Excellent communication and interpersonal skills, with a focus on empathy and cultural sensitivity
  • Competency in Te Reo Māori and Tikanga (Māori language and customs) or commitment to learning and growth
  • Certification as a registered nurse in New Zealand, with current annual practicing certificate
