**Job Overview**

We are seeking a highly skilled Clinical Lead to join our team of healthcare professionals dedicated to improving diabetes care in primary settings. This exciting opportunity is designed for experienced nurses who have a passion for delivering high-quality patient outcomes.

About the Role

As a Clinical Lead, you will be responsible for optimising oral therapy, conducting point-of-care testing, and referring patients for specialist insulin treatment when necessary. You will also exercise corporate nursing responsibilities, ensuring seamless integration with local and national diabetes guidelines (NG28).

Key Responsibilities

  • Identify sub-optimally controlled patients with Type 2 diabetes and develop effective management plans
  • Collaborate with multidisciplinary teams to deliver comprehensive care
  • Maintain knowledge and skills in the latest diabetes management practices
  • Exercise corporate nursing responsibilities, adhering to organisational policies and procedures

Requirements

To be successful in this role, you will need:

  • A minimum of 3 years' experience managing diabetes within primary care
  • Knowledge and confidence in providing education, support, and advice to patients with Type 2 diabetes
  • Skills and knowledge in optimising oral hypoglycaemics
  • A full driving licence
  • Current NMC registration
  • IT literacy, including primary care systems
