About This Opportunity

We're seeking a registered nursing professional with experience in MDS to oversee the accuracy and compliance of MDS assessments. As our MDS Coordinator (RNAC), you'll play a pivotal role in delivering exceptional patient care.

Key Responsibilities
  1. Lead the RAI Process to ensure precision and adherence to regulations.
  2. Partner with the multidisciplinary team to identify patient/resident needs and develop comprehensive care plans.
  3. Facilitate Care Plan conferences to foster collaboration among patients, residents, and their families.
  4. Promote education on the RAI Process and guarantee accurate coding of MDS assessments.
  5. Track Quality Measures to guarantee MDS assessments align with quality benchmarks.
  6. Maintain precise documentation and submit it in a timely manner to relevant authorities.
  7. E nsure compliance with federal and state regulatory requirements for Medicare and Medicaid.
Qualifications
  1. Two years of MDS experience preferred, but not required.
  2. Current registered nursing license in the state of employment.
  3. Familiarity with the MDS 3.0 framework.
