We are seeking a highly skilled Community Integration Specialist to join our team. As a Community Integration Specialist, you will work closely with individuals with brain injuries to promote their overall community integration.

  • Deliver collaborative client services including intake, service planning, life skills training, and coordination of other services to achieve client goals.
  • Document monthly progress toward identified goals.
  • Maintain ongoing communications with colleagues and external service providers.

Key Responsibilities

Provide crisis intervention and access emergency services to clients as needed.

Identify and address community integration issues for clients on caseload or as designated.

Transportation and training around utilizing transportation to individuals.

The ideal candidate will have strong analytical, numerical, and reasoning abilities, enthusiasm, motivation, open-mindedness, creativity, and the desire to thrive in a collaborative, team-based environment.

Requirements
  • BA/BS degree with 1-year experience working with individuals with TBI, ABI, or DD or 2 years experience working with individuals with TBI, ABI, or DD population.
  • Strong computer knowledge and excellent verbal and written communication skills.
  • A valid driver's license, access to an operational and insured vehicle, and willingness to use it to transport clients.
