We are seeking a Support Coordinator to join our team at Healthcare of New Zealand. As a Support Coordinator, you will play a vital role in facilitating the journey of families toward a valued life of their choosing.
Your primary responsibility will be to actively engage with families in a hands-on capacity and collaborate closely with them to provide assistance in daily living tasks such as grocery shopping, meal planning, preparation and cooking, laundry, light cleaning, and household support.
The key responsibilities of this role include:
- Creating a supportive environment for families, focusing on their aspirations and independence rather than waiting for a crisis to arise.
- Supporting young people to access education and achieve their goals.
- Observing and responding to health concerns or supporting the overall health and wellbeing of the young person.
- Tailoring supports to the family's unique needs and goals, adopting a holistic approach that encompasses all aspects of their life.
You will work closely with families to understand their needs and develop tailored plans to support their independence and participation in daily living.
In this role, you will have the opportunity to make a real difference in the lives of families and contribute to creating a society where everyone can live fully, have equal opportunities, and achieve their ambitions.
To succeed in this role, you should have:
- A passion for helping others and a kind, understanding attitude.
- Experience working with young people or families.
