Masonic Villages of Pennsylvania - Home Health, Hospice, and Home Care Campus - is a leading provider of award-winning care and services across the state.

We are seeking a skilled Home Health Social Worker to join our team. This role will play a vital part in assisting patients, families, and caregivers with emotional and practical issues while receiving care through the hospice or home health.

The ideal candidate will have experience in social work, preferably in hospice, home health, or medical settings. A Master's degree in Social Work (MSW) is required, as well as excellent communication and interpersonal skills.

Responsibilities:

  • Assure that social service activities are integrated and coordinated within the hospice and home health departments.
  • Perform psychosocial, bereavement, and safety assessments on all patients and families assigned.
  • Serve as a resource to patients and families for questions and emotional support.
  • Demonstrate knowledge of clinical social work and cultural competency in your work.

Requirements:

  • Graduated from a four-year college or university with a bachelor's degree in social work.
  • MSW required in accordance with Medicare Conditions of Participation for Hospice.
  • Personal computer experience required.
  • Hospice, Home Health or Medical Social Work experience preferred.
