We are seeking a dedicated individual to join our team as a Care Coordinator at Community Connections Supported Living Trust. Our vibrant and passionate organisation is driven by an unwavering passion for empowering people with disabilities to live the lives they choose.

The Role
As a Care Coordinator, you will play a vital role in guiding members of our community towards independence and mastery of essential life skills. Your responsibilities will include supporting individuals with personal care, meal preparation, creating support plans, and managing household tasks.
You will be part of a compassionate team providing 24/7 residential support, ensuring a safe, engaging, and empowering environment. Flexibility to work both day and night shifts is required.
About You
We are seeking a motivated and experienced individual who can keep pace with highly active individuals. Your exceptional time management and problem-solving skills will be invaluable as you navigate various situations.
Experience working with individuals who display challenging behaviours is especially desirable, particularly if you have a solid understanding of autism and intellectual disabilities.
