Job Summary

The Licensed Practical Nurse (LPN) will be responsible for administering personal care and routine skilled nursing care to patients in their homes. The ideal candidate will have excellent communication skills, be able to work independently, and possess a strong commitment to providing high-quality patient care.

Key Responsibilities
  1. Administer medications and perform treatments as prescribed by physicians
  2. Assess patients' physical and emotional needs and develop individualized care plans
  3. Provide education and support to patients and their families on self-care techniques and disease management
  4. Collaborate with interdisciplinary teams to coordinate patient care
  5. Participate in case conferences and contribute to patient care planning
Requirements
  • Graduate of an accredited LPN program
  • Licensed Practical Nurse (LPN) certification
  • BLS certification
  • Valid driver's license
  • Proof of current/valid car insurance
  • Knowledge and experience with electronic health records
