Clinical Rehabilitation Liaison Specialist

We are seeking an experienced Allied Health professional or registered nurse to join our dynamic Community Rehabilitation, Enablement and Support Team (CREST) in Christchurch. As a Clinical Rehabilitation Liaison Specialist, you will play a critical role in supporting patients to live well at home by providing short-term rehabilitation and enablement interventions.
Your primary focus will be on reducing the number of admissions of older persons to hospital and residential care through effective collaboration with referrers, clients, their families, and our CREST team. You will also support patients to leave the hospital and continue their recovery at home.
About the Role
This is a designated position on the Allied Health and Technical salary scale, requiring a current Annual Practicing Certificate in an allied health profession or a current registered nurse practicing certificate with no restrictions. Experience of working with older people is essential, and experience of working in the mental health sector would be an advantage.
In this role, you will establish rapid management plans focused on supporting rehabilitation and enablement through self-management, optimising patient goals, and independence. This will involve daily triaging of referrals to the allied health and nursing team as well as our colleagues in the home support provider services to ensure that each client receives timely and appropriate support.
You will act as an ambassador for our service, being calm and confident when dealing with challenging situations, and collaborate across an interdisciplinary team; communicating with various parties to ensure that we deliver a seamless service.
