Job Description

We are seeking a skilled and dedicated Clinical Assessor to join our Adult Community Therapy Service (ACTS) team in Canterbury. As a Clinical Assessor, you will be responsible for providing needs assessment and service coordination for clients funded by Long Term Supports-Chronic Health Conditions (LTS-CHC).

The ideal candidate will have at least 3 years of experience working within healthcare, with a high level of skill in clinical assessment and critical evaluation. Excellent interpersonal skills, including verbal and written communication, are essential for success in this role.

You will work closely with our experienced interdisciplinary team to develop coordinated care plans for LTS-CHC clients, complete InterRAI assessments, and communicate effectively with clients, family/whanau, and internal/external agencies.

Responsibilities:

  • Coordinate care plans for clients with complex chronic medical needs
  • Collaborate with interdisciplinary teams to deliver high-quality care
  • Communicate effectively with clients, family/whanau, and internal/external agencies

Requirements

  • At least 3 years experience working within healthcare
  • High level of skill with clinical assessment and critical evaluation
  • Excellent interpersonal skills, with a high standard of verbal and written communication
  • Ability to work autonomously, manage time well and prioritize workload appropriately
  • Must be a health professional registered to work in New Zealand (Social work, Occupational Therapy, Physiotherapy, Registered Nurse or related discipline)
